Registration & Immunizations
Welcome to Synergy! We’re happy to have you here and look forward to meeting you. Whether you’re new to the area or have a child ready for kindergarten, you’ll need to complete the registration process. Be advised, all incoming kindergarten students must be five years old on or before September 1 of the current school year in order to attend.
When you’re ready to enroll your child, please visit our central office at 2701 W. Bethany Home Road (across from Savers). Be sure to bring the following documents:
- Completed enrollment packet (or complete the online form prior to your visit)
- Child’s original birth certificate
- Immunization records
- Proof of residency
- Lease agreement/mortgage statement
- Utility bill
- Property Tax statement
- Payroll check stub
- Withdrawal slip and final report card from previous school (if applicable)
- Notarized proof of guardianship (if applicable)
- Picture ID

Synergy Public School is a free, public charter school serving students living in urban Phoenix. In the event more students enroll at Synergy than space and seating allow, we will hold an enrollment lottery.
In accordance with Arizona law, we give priority to siblings of current students and children of staff members but then hold drawings to fill all remaining seats in each classroom. We place applicants not selected by lottery on our waiting list.
